Prior to the news report of fentanyl laced heroin we at the UFD found that one dose of NARCAN was ineffective with many recent overdoses. We discovered through certain channels the info about the fentanyl. Any others have the same experiences?

Capt, your right. I've had the same experience with the heroin as of late. In one wkend, I personally was dispatched to 6 or 7 overdoses and had to use at least two doses of Narcan on each pt. And in one had to use a third. I also found out that nasal Narcan was doing absolutely nothing.

I think it could be beneficial to see if the DOH can roll out protocol for a more concentrated dose of narcan in hopes that one shot will be enough to kick a substantial amount of the opiate molecules off the receptors and awaken the pt. In addition, certain ambulance companies who run a lot of OD calls could potentially save a decent amount of money each year considering each of the current doses are ~ 20-50 bucks. Stronger doses could decrease the amount of units used...Maybe it's just wishful thinking.

Fentanyl is ridiculously powerful, like 50 something times greater receptor affinity than Heroin and twice that than morphine. who knows what the synergistic effects of fentanyl and heroin are. And who knows what these people are doing to extract the fentanyl, in what concentration they are mixing it, etc..

I did an overdose the other day with the fire department, It took 2 rounds of internasal narcan, plus a full 2mgs of IV narcan for a return of respirations, the patient was still out but it just shows they are making it stronger and stronger.
